Please follow the instructions in the link provided below when reporting a Send-Off of a player, coach, or substituted player. It is very important that this report is completed on-line within 24 hours of the actual send-off. Also, please note that the referee does not keep the player pass. However, you need to make certain that you have recorded all necessary information from the player's or coache's pass. Important information includes full name, club name, team name, player ID number. Referee is also required to contact the league's referee assignor and inform him/her of the situation.
