Fall 2010 Recreation Registration

Registration Period: Registration for the spring season will be open from May 1, 2010 thru June 19, 2010

Late Registration Period: Late registration June 20, 2010 thru July15, 2010 space available, no coach or friend requests. A $25.00 late fee is applicable and only available online. Mailed applications with postmarks after June 19^th will be returned

Registration Deadline: June 19, 2010

Dates for fall: September thru November 2010 (these dates are tentative)

Who can play? Boys and girls ages 4 thru 13

Fees: $85.00 U6 and U8 Players………………..7-31-07 to 8-1-02

$140.00 U10 thru U14 players…………...7-31-02 to 8-1-96

Registration Requirements: Registration forms will be available for download once the registration period has opened. Hardcopy forms will also be made available.

To properly complete the registration process:

	The player’s name must match the name on the birth certificate.
	Be sure to sign parent agreement and consent for medical treatment.
	Registration must include a non-returnable copy of a certified birth certificate, passport or alien card. (Hospital and baptismal certificates are NOT acceptable.)
	Returning players that registered online last season ONLY, Birth Certificates are not required, unless you are moving down from competitive.

MAIL (Do Not Hand deliver!): Registration form, birth certificate, and check or money order to: OVYSL, 19081 Clayton Ave, Morgan Hill CA 95037. DO NOT SEND CASH.

Teams are filled on a first come first served basis. The league depends on parent volunteers and, therefore, can only accommodate players based on coaches available.
