RECREATION FALL 2008 REGISTRATION-DIVISION IV

Registration Period – Registration for the fall season will be open from May 1, 2008 Through July 1, 2008. 

Dates for Fall – September 6 through November 8  (these dates are tentative)

Who Can Play - Boys & Girls ages 4-13
(Birth dates Aug. 1, 1994 to July 31, 2005)

Fees -     $85 for U5 - U8( Players 8-1-2000 Through 7-31-05.) $135.00 for U10- U14 ( Players Born before  8-1-2000)

Registration Requirements – The registration form will be available for download once the registration period has opened.  Hardcopy forms will also be made available at The Morgan Hill Bowl, 650 Tennant Station, Morgan Hill. Or  Soccer ‘N’ More, 7001 Monterey Rd. Gilroy.  

To properly complete the registration process, the player’s name must match the name on the birth certificate. Be sure to sign parent agreement and consent for medical treatment. Registration must include a non-returnable copy of a certified birth certificate, passport or alien card. (Hospital and Baptismal Certificates are NOT acceptable.)  Include a small colored photo of player (1" X 1½"). Player may not be wearing sunglasses or hat.

MAIL (Do not hand deliver!) registration form, birth certificate, photo and check or money order to OVYSL, 19081 Clayton Ave, Morgan Hill, CA 95037. Do not send cash.

Please follow the instructions carefully & submit all required items.  Failure to do so will delay player registration!

Deadline – July 1, 2008

Teams are filled on a first come, first served basis. The League depends on parent volunteers and, therefore, can only accommodate players based on coaches available.
