Orchard Valley Youth Soccer League Forms

bullet OVYSL Scholarship Form Each team is allowed a maximum of two scholarships per season.  These scholarships are available for families that are considered "hardship" cases that have trouble paying the Fall registration fees.  Applicants are asked to fill in the attached form and submit a check for $75.  The league covers the rest of their fees.  The team coach/manager is responsible for using his/her discretion as to who requires financial assistance and subsequently submitting the scholarship form to the league.
bulletClass 1 or Class 3 coaching application.   You need to fill out the following form found here and deliver it to the league address on that form. You must also possess a USYSA-sanctioned coach's license.
bulletThe Orchard Valley Youth Soccer League is now accepting resumes from anyone interested in serving on its Board of Directors. The application form is here. Please indicate which position(s) you might be interested in and mail along with your resume to P.O. Box 891, Morgan Hill, CA  95038 or FAX it to 779-6696.
